Hearing Checklist

Are you concerned about your child's hearing?
Complete the following checklist to see if your child needs a referral for a hearing test.

• Birth
• 0-3 month
• 4-6 months
• 7–12 months
• 1-2 years
• 2-3 years
• 3-4 years
• 4.5–5 years

If you answered NO to any of these statements you should consult your doctor about a referral to have your child's hearing tested by an Audiologist.
